Sew Simply Monkey Order Form

Use this form to place special orders such as matching tops, pants,
dresses or specialty items.
Please enter, if possible, any measurements.

When you place an order, you will receive an email confirming your
order and a PayPal invoice relating your total order including
shipping/handling.

You can pay the invoice with Paypal, or call in your credit card if you
do not use Paypal. Or you may mail a check or money order.

Shipping/Handling fees:
Minimum fee 3.95 & up

Priority Mail: minimum 4.75
Insurance(Optional) 1.70

No overnight or 2nd air.

No S/H fees on orders over 100.00

Orders are prepaid plus S/H fee.

We accept check or money order.(Personal
checks must clear before order is shipped)

You do not have to be a member of Paypal to
purchase with a credit card.

Mastercard, Visa or Discover by call-in only
20.00 dollar minimum.

Florida residents must pay sales tax.

Exchange or Refund Policy: You have up to 15 days after you receive your custom order to
either exchange it or request a refund. Items returned must be in the same condition in which they
were received by you before exchange or refund can be issued.
Clearance items are non-returnable, non-refundable due to health concerns.
